Media Release Form

This form gives SISD and SHS permission to use any photo or video taken at choir events throughout the school year on the Sunnyvale HS Choir Website or social media platforms. This form also gives the photographer SHS class of 2017 graduate Cameron Justice and his production company CJustice Productions permission to use any photo or video he takes at choir events throughout the year on his personal business website or social media platforms for the purpose of advertising and portfolio building. By signing this form you are granting permission to SISD, SHS Choir, Cameron Justice and his production company CJustice Productions permission to use any and all photos and videos taken of you throughout the school year at official choir events on social media and websites. No images or videos will be distributed or used in any way not deemed appropriate by SISD. Follow the QR code to fill out the form!!
